I/WE, the parent(s) or guardian(s) of the above named candidate, hereby give my/our approval for his/her participation in any and all  sports activities, practice, and transportation of team to games during the current season. I/WE, assume all financial responsibility for payment of medical expenses for the above named applicant. The MOUNTAINVILLE MEMORIAL ASSOC. Inc. assumes no liability or cost in the event or illness of any kind due to the participation in the above mentioned activities.*

I/WE do agree to promptly return all uniforms and equipment in the same condition as received, except for normal wear for the sport at  the end of season or when my/our child no longer is participating. Damage or loss of the above uniform and/or equipment will result in  the parent(s) or guardian(s) payment for replacement of same, PLUS, any court, filing and mailing fees incurred. Any athlete voluntarily  quitting their sport i.e., during the season, after the first practice or after uniforms *
